Humana Medical Benefits 2007-2008
Medical Insurance
All permanent employees are eligible. New employees have 30 days from their date of hire to enroll. The effective date is the first day of the following month.

The annual renewal date for insurance is January 1st. Current employees who choose to change their enrollment status may do so at the open enrollment period. The school district contributes $225 for individual and family coverage.* The employee pays the difference.

*This does not apply to retire-rehire status employees.

Disability Insurance
American Fidelity Insurance Company is the disability insurance provider. Benefits are tailored for each employee, and premiums depend on current salary, amount of disability benefit, the waiting period you select, and your age.

Depending on plan elected, disability benefits are paid after a disability occurs or on a day to day basis for illness and hospitalization. Please refer to the plan you have chosen.

Dental/Vision Insurance
All permanent full-time employees are eligible. New employees hve 30 days from their date of hire to enroll. The effective date is the first day of the following month. The premiums are paid by the employee.

Life Insurance
Employee life insurance in the amount of $20,000 plus an additional $20,000 for accidental death is provided to all full-time employees up to age 65*. The premium is paid for by the District. Additional coverage for your family can be purchased also. The employee pays the premium.

*Personal Life and AD&D Insurance will be reduced as follows:

  • At age 65, benefits will reduce by 35% of the original amount;
  • At age 70, benefits will reduce an additional 20% of the original amount;
  • At age 75, benefits will reduce an additional 15% of the original amount;
  • At age 80, benefits will reduce an additional 10% of the original amount.

Flexible Spending Account
Available to all employees, the Flexible Spending Account is a voluntary plan that allows you to pay selected expenses on a pre-tax basis. It can be used in two different ways:

1) Medical and dental expense reimbursement (examples: eye glasses, contacts, braces, elective surgery, Rx co-pays)

2) Dependent care reimbursement.

Employee Wellness Program
The district provides a voluntary health and fitness program to all employees at no cost. Some of the activities include:

· Health assessments and flu shots
· Walk/run events
· Golf, softball & tennis tournaments
· Water aerobics, Yoga, Weight Watchers
· Health & fitness workshops
· Discounts at area gyms
· Monthly awards

Service Awards
To show appreciation for continued service, employees are recognized and receive a service award for every five years of employment with the district.

Workers Compensation
All FBISD employees are insured by workers’ compensation. Seventy percent of wages are paid after an initial seven day waiting period.

Tax Sheltered Annuities 403(b)
Tax sheltered annuities are designed for the enhancement of your retirement income. FBISD encourages their employees to review these options. Employees may choose their provider; however, all enrollments must be submitted through First Financial to insure compliance with the IRS.

Cancer and Dread Disease Insurance
American Fidelity Insurance Company is the provider for our cancer/intensive care/hospital indemnity. American Fidelity provides our district with supplemental coverage that will pay in addition to your health insurance. Benefits are paid directly to the policyholder.

After School Care
After school care for employees’ children who are enrolled in the ECC, Primary, Elementary and Intermediate schools is available free of charge. Check each campus for time schedules.

The District hereby reserves and retains the right to amend, alter, change, delete, or modify any of the provisions of this publication at any time, and from time to time, without prior notice in any manner that the administration or the Board of Trustees deems to be in the best interest of the District.

State Personal Leave
Monthly full time employees shall earn state personal leave at the rate of one-half workday for each 18 workdays of employment, up to the statutory maximum of five (5) workdays per fiscal year. Leave will be credited at the beginning of the fiscal year (September 1).

Bi-weekly full time employees shall earn state personal leave, in hour increments, to be recorded on the first of each month beginning September through June, up to the statutory maximum of five (5) workdays per fiscal year. The rate of accrual shall be one-half workday for each 18 workdays of employment.

Local Sick Leave
Local sick leave shall accumulate with no maximum for all employees.

Monthly full time employees shall earn local sick leave, at the rate of one-half day for every 18 workdays, to a maximum of seven (7) days per fiscal year. Leave shall be advanced at the beginning of the school year.

Bi-weekly full time employees shall earn local sick leave, in hours, to be recorded on the first of each month up to a maximum of seven (7) days per fiscal year. Rate of accrual shall be one-half workday for each 18 workdays for the first 10 months and one workday for each additional 18 workdays for the 11th and 12th months.

Jury Duty
Employees shall be granted leave with pay without loss of accumulated leave for jury duty and court appearances in response to a subpoena.

Bereavement Leave
State leave and/or local sick leave shall be used for a death in the “immediate family”. Leave shall not exceed five workdays per occurrence.

Military Leave
An employee who is a member of a military or state guard/reserve unit will be granted a paid leave of absence without loss of any accumulated leave for authorized training or duty orders.

Extended Leave Benefits
The District provides extended sick leave for employees who are not receiving workers’ compensation benefits and have exhausted all personal and sick leave. The maximum number of days eligible varies according to years of employment.

Family and Medical Leave
Additional extended leave is provided under the Family Medical Leave Act (FMLA). Medical certification of an illness or disability of an employee, spouse, parent, or child must be certified by a physician or practitioner prior to leave being granted.

Leave Donations
Employees are eligible for leave donations when they are needed to care for a seriously ill, immediate family member. Illness must qualify under the FMLA guidelines. When the employee no longer has personal and sick leave available other employees may donate leave to them. The following guidelines will be followed:

•Employees must have exhausted all state and local leave, and vacation before being eligible to receive donations.

•Donations are voluntary contributions by district employees for a specific individual who meets eligibility requirements.

•Donations are non-refundable and non-transferable.

•The employee donating leave must maintain 5 local sick leave days after donations.

•An employee shall be allowed to contribute no more than 4 half-days per fiscal year.

•Contributions will be in half-day increments only.

Sick leave buy back Program
The District shall operate a limited leave buy back program for employees who have accumulated excess local leave at three different options.

1) Annual Attendance Incentive - employees who have accumulated in excess of 20 days of accumulated local sick leave. Transactions under this option must be a minimum of 5 days and a maximum of 15 days.

2) Separation from employment - employees who resign from employment in the District, in good standing, are eligible for buy back of unused local leave days in excess of 15 days, at the rates described in the employee personnel manual.

3)Retirement - employees who retire through the Teacher Retirement system are eligible for buy back of unused local and state leave days, at the rates described in the employee personnel manual.
